**Job Title**: Health Outcomes Coordinator
We are seeking a proactive and compassionate Health Outcomes Coordinator to join our passionate team. In this pivotal role, you will oversee health outcome tracking processes, ensuring the completion of health screeners and supporting data-driven clinical assessments.
**Key Responsibilities**:
- Conduct routine calls to administer health screeners and ensure timely completion among our construction industry members.
- Follow up diligently with members to facilitate completion of screeners and address any concerns.
- Compile and analyze screener data to produce insightful reports for our clinical team.
- Collaborate closely with clinicians to provide accurate data for personalized health assessments.
- Maintain meticulous records of member interactions and screener completion status.
- Proactively identify trends and opportunities for improving our screening and assessment processes.
**Requirements**:
- Bachelor’s degree in a health-related field or equivalent practical experience.
- Proven track record in healthcare administration, patient outreach, or similar roles.
- Exceptional organizational skills with the ability to manage multiple priorities effectively.
- Strong interpersonal and communication skills, with a compassionate approach to member care.
- Ability to thrive in a dynamic environment and contribute effectively within a collaborative team setting.
**Preferred Qualifications**:
- Familiarity with electronic health records (EHR) systems.
- Knowledge of mental health and substance use screening protocols.
- Certification or training in healthcare management or related fields.

Pay: $22.45-$25.00 per hour
