Position Summary:
Responsibilities include:

- Liaise and network with community agencies, leaders, and members around chronic disease prevention and management
- Outreach to various healthcare providers, internal and external
- Promote diabetes education and other chronic disease-related program, including tele-ophthalmology - in person, on social media, FHC’s website, in community events and fairs, to encourage referrals, recruit program participants and create general community awareness about FHC and the services we provide
- Support the delivery of chronic disease education sessions on/offsite
- Coordinate the full team’s chart audit, program evaluation and quality improvement initiatives
- Gather and organize and analyze data (including health equity), maintain accurate records, complete and submit regular progress and evaluation reports
- Address queries and answer phone calls and correspondence pertinent to the Chronic Disease services and programs
- Support the tracking of emergency food cards, transportation support and interpretation services across the full team
- Complete all administrative duties associated with this role
- Cross-collaborate with colleagues and providers within the organization
- Assist in the recruitment, training, supervision and engagement of volunteers
- Participate in internal/ external committees, as required
- Promote and support attainment of organizational vision, goals and objectives
- Other duties as directed by Manager, Chronic Disease and Mental Health or designate

Qualifications, Skills and Proficiencies:

- University degree and/or a combination of post-secondary education and experience
- Demonstrated community outreach and engagement skills
- Able to work with people from diverse communities
- Team player with demonstrated interpersonal skills; with high ability to exercise good judgment and tact
- Proven presentation, facilitation and organizational skills
- Proficient in MS Office (Word, Excel, PowerPoint, Publisher and Outlook) and Canva; knowledge and experience in PS Suite (electronic medical records system) an asset
- Proven capacity for initiative taking and ability to carry projects to completion with minimum supervision; ability to assess priorities and meet deadlines
- Excellent written & verbal communication skills
- Ability to collect and analyze community data for the use of program promotion and development (e. g. statistics and health equity data)
- Demonstrated ability to access community resources: familiarity with the communities being served across the three sites: Flemingdon, Thorncliffe and Fairview
- Ability to speak another language other than English an asset
- Vaccines (COVID-19 and others) are a requirement of the job pursuant to the Ontario Human Rights Code.

**Job Type**: Part-time

**Salary**: $29.14-$34.65 per hour

Expected hours: 21 per week

Schedule:
