Team collaboration: Use a collaborative, interdisciplinary approach to care and services. Holistic care: Support a restorative approach to health, offering interventions, health coaching, and self‑management support. Digital health: Utilize our digital platforms for care delivery, collaboration, documentation, and intervention. Qualifications Registered member in good standing with the College of Occupational Therapists of Ontario. Ability to travel within the community – valid driver’s license and insurance required. Person- and family-centered care focus with passion and empathy. Good judgment and critical thinking in client care decisions. Excellent communication skills, thriving in both team settings and working independently. Advocacy for home care: value the role of a therapist in enabling clients to remain at home when it aligns with their personal goals.
