We are funded by the Ministry of Health, Ontario Health, Ministry of Community, Children and Social Services, the City of Toronto and many generous donors and foundations.about the role:The Case Manager engages in a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet the comprehensive health needs of 2SLGBTQ youth and young adults 14-29.Working at SherbourneAt Sherbourne Health, we care about our staff and recognize that our biggest strength is our people. We supplement this by creating a low-barrier work environment built on equity and respect, while working together in caring, cohesive teams and providing opportunities for advancement.we believe in work-life balance and offer:Membership in Healthcare of Ontario Pension Plan (HOOPP).Up to four weeks of vacation per year (to start).Up to four personal days per year.An extended healthcare and dental package.Life insurance.An employee assistance plan.A generous professional development plan: up to $1,300 to be used towards learning activities.Up to six fully paid professional development days per year – related to organizational objectives.Responsibilities and Tasks:Case ManagementPerforms intake assessments, works with service users to identify goals and develops a smart plan that supports the service user to achieve their goals.Facilitates coordination, communication, and collaboration with service users, as well as internal and external partners to support service user goals.Determines risk factors for threats of harm to self and others, mental health issues and substance use and prioritizes support based on urgent needs.Provides brief supportive counselling to service users in distress, when necessary.Facilitates and participates in case coordination meetings to facilitate service user access to care,as needed.Evaluates outcomes of plans and facilitates moving on process.Maintains records in the electronic medical recording system, in accordance with Sherbourne policies and procedures and privacy legislation.Workshop and Group FacilitationPlans and organizes workshops and/or short-term group programming.Conducts outreach to targeted participants for workshops and short-term groups.AdministrationParticipates in team and organizational planning processes.Operates within established policies and Maintains service statistics.Participates in ongoing evaluation of programs and service delivery.Participates in Sherbourne meetings, committees, special events,etc.Performs administrative tasks as required.To thrive in this role, you’ll neededucation/experience:Post-secondary education in social work or equivalent formal trainingAt least 3 years’ work experience with 2SLGBTQ youth and young adults from communities that experience systemic marginalization/ oppression or an equivalent blend of education and experience.Case Management Certificate from a recognized educational institution, and current registration with the Ontario College of Social Workers and Social Service Workers are an asset.Experience using Electronic Medical Recording systems, Accuro preferred.skills and abilities:Must be service user focused with excellent problem-solving skills and ability to deal effectively with crisis and conflict.Professional communication skills and ability to adapt communication style to work effectively with colleagues, partner agencies and service users.Knowledge of and ability to develop relationships with community resources and agencies.Ability to work both collaboratively in an interdisciplinary framework and independentlyDeveloped time management and prioritization skills.Thorough knowledge and experience working from a harm reduction and trauma informed approach.Excellent assessment and engagement skills and sound judgement.Demonstrated experience maintaining professional approach.Demonstrated experience maintaining professional boundaries.Experience working with 2SLGBTQ youth and young adults and identification with, or strong connections to 2SLGBTQ communities.A strong understanding of the social determinants of health.conditions of employment:Offer of employment is contingent upon satisfactory outcome of background checks e., professional references, education verification, criminal background check, and vulnerable sector check.Understands and works within the Occupational Health and Safety Act (OHSA), Personal Health Information Protection Act (PHIPA), Accessibility for Ontarians with Disabilities Act (AODA), the Ontario Human Rights Code and other relevant legislation.Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, ) by taking all reasonable precautions and working in compliance with SHC related policies, health and safety legislation and best practices.working conditions:Ability to work a fixed schedule and have flexibility to work evenings and weekends.to apply: Please forward a cover letter and resume quoting Job ID # CPCBI CM YRW 25-0731 to myjob@sherbourne.on.ca by 5:00 pm on August 22, 2025.Sherbourne is dedicated to building an organization that reflects the diversity of our clients and the communities we serve.
