Hoffmeister Building1755 West 1st Avenue, Suite 142Vancouver, BC V6J 0G7 Free parking is available on-site About the Transition Centre The Mainland BC Military Family Resource Centre (MFRC) is a non-profit, community-based organization dedicated to supporting Canadian Armed Forces (CAF) members, Veterans, and their families across Mainland British Columbia. As part of the national Military Family Services Program (MFSP), the MFRC delivers programs and services that strengthen family resilience, promote well-being, and support families through all stages of the military and Veteran life cycle. Working in close partnership with the CAF, community organizations, and service providers, the MFRC offers family‑centred supports that recognize the unique demands of military life, including relocation, deployment, illness or injury, and transition to civilian life. About the Transition Centre The Canadian Armed Forces Transition Group (CAF TG), in partnership with Veterans Affairs Canada and a network of service partners, delivers professional, personalized, and standardized transition services to CAF members, veterans and their families, with a particular focus on those who are ill or injured as they transition to civilian life. The Vancouver Transition Centre is the local hub of the CAF TG, offering an integrated, one‑stop service model that provides transition services and casualty support to ill and injured CAF members and their families. Job Summary Embedded within the Transition Centre, the FLO operates under a national mandate, supporting families during medical release, transition planning, and adjustment to post‑service life. The role delivers short‑term counselling, emotional stabilization, crisis intervention, and evidence‑informed psychosocial support to address transition‑related stressors such as illness, injury, identity change, system navigation, or death related to service. The FLO works within clearly defined role boundaries, distinct from MFRC Family Wellness Counsellors, and follows established protocols. The FLO participates in interdisciplinary teams supporting medically releasing members and their families. When longer‑term or specialized care is needed, the FLO facilitates warm referrals to appropriate community or MFRC services to ensure continuity of care. Practice is guided by professional ethics, privacy legislation, and MFSP standards, with caseloads managed in consultation with the Executive Director. Key Responsibilities Psychosocial Support (Primary Function): Provide evidence‑based, short‑term, transition‑focused psychosocial counselling and emotional stabilization to families of medically releasing CAF members, and to families of the fallen Support families coping with illness, injury, identity changes, and the psychosocial impacts of medical release Participate in regular individual and group clinical supervision Transition‑Specific Navigation: Assist families in navigating relevant resources related to medical release and transition Deliver transition‑specific navigation unique to the FLO role and distinct from general MFRC services, including support through the medical release process and coordination with VAC Case Managers and Transition Advisors Coordinate warm hand‑offs to the Veteran Family Program (VFP) Coordinator and other MFRC services, and community providers as families transition post‑release Collaboration & Systems Coordination: Collaborate with VAC Case Managers, CAF medical staff, Personnel Support Programs (PSP), and community partners while adhering to professional boundaries and chain of command protocols Contribute to interdisciplinary planning to reduce service gaps and support coordinated care Maintain clear understanding of scope of practice, role boundaries, and organizational reporting structures Documentation & Accountability: Maintain accurate, confidential documentation, including in Penelope CRM Contribute to performance measurement, reporting, audits, and site visits as required Participate in professional development and team initiatives as required Qualifications Required: Registered Clinical Counsellor (RCC or CCC) or a Registered Social Worker (RSW) in good standing with the appropriate provincial regulatory body Minimum two (2) years of post‑graduate clinical experience Demonstrated experience in crisis intervention and family‑centred psychosocial support Strong interpersonal and communication skills Commitment to ethical practice, collaboration, and ongoing professional development Ability to work independently and effectively within a multidisciplinary team Demonstrated capacity to maintain professional boundaries, with clear understanding of scope of practice, chain of command, and organizational protocols Ability to collaborate effectively in a complex, multi‑stakeholder environment Assets Experience working with military or Veteran families Strong understanding of military culture and the impact of illness, injury, and transition on families Knowledge of CAF systems, VAC, and provincial/community‑based resources Experience delivering short‑term, solution‑focused therapeutic interventions Bilingualism (English/French) What We Offer Health and dental coverage (following successful completion of 3‑month probationary period) Access to Employee and Family Assistance Program Three (3) weeks paid vacation annually (pro‑rated based on FTE) Paid professional registration fees Regular clinical supervision (group and individual) Opportunities for professional growth, mentorship, and leadership development Flexible scheduling that supports work‑life balance Screening & Security Requirements The successful candidate must complete and pass a Criminal Record Check for Working with Vulnerable Adults and Children through the BC CRRP.
