Intake and Care Coordinators are the primary care coordinators within the FCC. They provide intake and service coordination functions for parents/caregivers and their children or youth with support needs who are accessing services through the FCC. Hours / Schedule 1 Full‑Time, Temporary position to cover a leave from Jan. 2026 – end of April 2026. Averaging 37.5 hours per week. Responsibilities Intake and Care Coordinators will provide individualized service planning support for families to address identified needs and coordinate access to FCC services: Conduct brief screening and assessments to identify the specific service needs of each child, youth, and/or family. Provide information regarding child development, available services within the FCC, and other available resources in the community. Develop a goal‑oriented Individualized Service Plan (ISP) to address service needs, which may include services to be provided by the Intake and Care Coordinator and/or other practitioners of the FCC. Connect the child, youth or family and support the transition between relevant FCC services. Provide needs‑based screening and developmental monitoring throughout service engagement and update service goals as necessary in collaboration with other FCC practitioners. Assist families as needed with appointment reminders, coordination of services, needs identification, advocacy, and organizing consistent Integrated Case Management meetings with child/youth and family’s interdisciplinary team. Remain connected with regular individual meetings as the family’s consistent point of contact throughout their service. Implement required case management and clinical records. Other responsibilities include: Building relationships and connections within the community to improve service integration. Applying culturally safe and agile, trauma‑informed, relational and person/family‑centered modalities. Preferred Education and Experience A bachelor's degree in a Social Services or Human Services discipline (e.g., BSW, CYC, BA in Psychology, etc.) with a minimum of 2 years relevant experience is preferred. Other levels of qualifications, including position‑specific experience, knowledge, and skills, may be considered. Desired Skills Excellent interpersonal and verbal/written communication skills. Detail‑oriented, organized, and efficient. Skill at managing multiple concurrent projects, assignments, and tasks requested from multiple sources. Capable of working independently and frequently under pressure. Ability to learn and apply multiple complex IT platforms to manage your casework. Other Requirements Need a valid BC Driver’s License, regional insurance, and a reliable personal vehicle. Must undergo a screening process, including a criminal record search. Wage / Benefits $30.25 – $31.75 per hour based on relevant experience and education. Upon successful completion of a probationary period, full‑time employees (minimum 20 hrs per week) are eligible for a full benefit package (including dental plan, vision coverage, life insurance, extended medical coverage, long‑term disability plan, and RRSP or TFSA contributions). New full‑time employees are eligible for 3 weeks paid vacation to start and paid sick days as described in agency policy.
