This is an exciting opportunity to work within a large, well-established practice that values innovation, teamwork, and high-quality patient care.The successful candidate will play a key role in delivering autonomous clinical care, supporting our multidisciplinary team, and contributing to ongoing service development.Main duties of the jobProvide high-quality clinical assessment, diagnosis, treatment, and management of patients presenting with a wide range of undifferentiated and undiagnosed conditions.Undertake acute illness / disease assessment and management, including prescribing and reviewing medications within scope of practice.Support urgent on-the-day demand alongside the duty team.Deliver proactive, patient-centred care for long-term conditions in collaboration with the wider clinical team.Request and interpret investigations, make referrals, and liaise with other health and social care professionals as appropriate.Contribute to clinical governance, quality improvement, and the development of care pathways.About usStainland Road Medical Centreis a forward thinking, innovative 5 Partner Practice. We are seeking an enthusiastic registered Practice Nurse to join our team and play a pivotal role in patient care.Supportive and friendly environment.A well established training practice with opportunities for training and development.Competitive pay.Job responsibilitiesUndertake patient consultations in surgery, no home visiting expectationsWork independently to assess, diagnose, plan, implement, and evaluate treatment plans within scope.Prescribe and review medication in accordance with local and national guidelines.Identify, manage, and refer patients appropriately, ensuring safe and effective clinical decision-making.Maintain accurate and contemporaneous patient records.Participate in clinical meetings and contribute to the ongoing development of practice services.Person SpecificationQualificationsRegistered Nurse with valid NMC registrationMSc or PG Diploma in Advanced Clinical Practice (or equivalent experience)Evidence of ongoing professional developmentExperienceProven ability to assess and manage acutely unwell and chronically ill patients in a primary care settingExperience in practices using triage, prescribing, and working within a multidisciplinary teamExperience of working in general practiceDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
