They will be responsible for the assessment, planning, implementation, and evaluation of clinical care within the Early Psychosis Intervention (EPI) program.The Early Psychosis Intervention (EPI) program provides standardized, evidence-based treatment through a coordinated care model - the NAVIGATE Program for Early Psychosis Intervention. This position specifically focuses on the Supported Education and Employment (SEE) component of NAVIGATE, helping clients pursue educational and vocational goals as part of their recovery journey.What You Will DoEstablish supportive, recovery-oriented relationships with clients, providing assessment, psychoeducation, and case management support to clients, their families, and support systems.Collaborate within a multi-disciplinary team and with EPI partners to ensure coordinated and comprehensive care.Support clients in developing education and employment goals aligned with their interests; assist with job placement or enrollment in educational programs and provide follow-along support to promote success.Develop individualized holistic crisis plans in collaboration with clients and their supports to maintain wellness.Create and implement recovery plans to guide goal-setting and support progress toward recovery.Assist clients in identifying and working toward educational, vocational, and life skill development goals.Consult, advocate, and liaise with community partners to support clients in achieving their goals.Monitor mental health and apply appropriate interventions to promote stability and well-being.Provide support with Community Treatment Orders and ongoing legal matters as required.Participate in case conferences, advocating for coordinated care and comprehensive support.Complete referrals and collaborate with internal and external teams, including post-graduation follow-up.Maintain accurate documentation, client records, statistics, and outcome measures in accordance with CMHA Peel Dufferin policies and procedures.Plan and facilitate psychoeducation groups and social/recreational activities.This posting description indicates the key responsibilities and core functions of the position.
