Founded in 1981, CEDC’s core program areas are the development and construction of rental housing for very low‑ and low‑income households; property management; and resident and community services. CEDC provides high‑quality services to individuals experiencing homelessness. Position Summary Under the supervision of the Director of Resident and Community Services, the Case Manager performs a variety of functions vital to supporting each property with low‑income, previously homeless, disabled, and mentally ill residents. The role focuses on permanent supportive housing residents and others with special needs, assisting with housing retention and stability, community resource access, and behavior management. Major Duties and Responsibilities The Case Manager provides direct, on‑property assistance and support to residents experiencing a housing crisis and homelessness through clinical engagement and intensive case management services. Key responsibilities include: Conduct psychoeducation on social and living skills, provide supportive counseling, and crisis interventions. Develop and promote housing readiness and provide trauma‑informed, motivational interviewing, and harm‑reduction care. Engage with interdisciplinary teams to provide services to high‑needs homeless/formerly homeless individuals with active mental illness, substance abuse, and co‑occurring conditions. Assess psychosocial functioning and needs, conduct assessments, coordinate services, referrals, and linkages, and develop individualized case plans and treatment goals. Maintain client records, mandate reporting, and meet with clients weekly to re‑evaluate case plans. Position Responsibilities (Detailed) Develop and implement housing plans and participant budgets; re‑evaluate case plans as needed. Coordinate with internal and external partners (Housing Authority, Ventura County Behavioral Health, Veterans Administration, etc.) to facilitate optimal outcomes. Maintain scheduled contact with housing clients based on status and needs. Enter data and provide annual reports to HCD and CalVet, adhering to HMIS database standards. Document and maintain records in compliance with HIPAA, HMIS, HCD, and TCAC mandated services. Apply trauma‑informed, motivational interviewing, and solution‑focused approaches. Ensure data and record‑keeping compliance with specific plans. Conduct initial meetings and assessments to build rapport and evaluate housing needs, history, barriers, and supportive services. Engage participants in tailored Housing Stability Plans to determine permanent housing interventions and service needs. Connect residents to community resources (benefits advocacy, food pantries, employment services, mental health). Seek consultation through regular case conferencing for crises (child abuse reporting, suicidal or homicidal risk assessment). Facilitate access to services supporting housing readiness (childcare, credit repair, legal aid, housekeeping, money management, tenant rights). Coordinate with Property Management to secure necessary documents (valid ID, income verification, bank statements). Meet with residents weekly to review progress, housing tenant rights, and rental agreement compliance. Assist clients in applying for permanent subsidies or low‑income housing assistance programs (Section 8 certification, EHAP, CalWorks Homeless Move‑In Assistance Program). Participate in supervision, agency, and community meetings as directed by supervisor. Complete VHHP Annual Reports to California Departments of Housing and Community Development and Veterans Services. Required Skills, Knowledge, and Abilities Strong knowledge of social service, public assistance, legal, corrections, health care, substance abuse, veterans, mental health care, and other systems associated with the target population. Demonstrate empathy and a non‑judgmental attitude when engaging with caseload. Advocate effectively on behalf of low‑income and/or the unhoused. Work with families and individuals experiencing homelessness or risk factors from a trauma‑informed and strength‑based approach. Flexibility and ability to adapt and thrive in fast‑paced, challenging environments. Knowledge of case management services and clinical approaches of Trauma‑Informed Care, Motivational Interviewing, and Harm Red‑Ution Models (preferred). Knowledge of fair housing laws and tenant relations. Strong multi‑tasking ability in a mission‑driven organization with high ethical standards. Familiarity with the Coordinated Entry System (CES), HMIS/ICMS implementation. Strong computing skills in Microsoft Office Suite (Outlook, Word, Excel, Power Point). Bilingual in English/Spanish preferred. Required Education & Experience Master’s degree in social work (MSW), marriage and family therapy (MFT), psychology, or related field with 1‑2 years of related experience OR Bachelor’s degree from an accredited institution in social work, psychology, or related field plus 3‑5 years of experience. Licenses & Other Requirements A valid California driver’s license and proof of automobile liability insurance.
