Senior Support Coordinator – Homewood Health Centre Make a difference in a growing, innovative mental health organization and centre of excellence in people‑centered care. What You’ll Be Doing Provide support coordination for evidence‑based, complexities within the senior population with serious mental health and concurrent disorders Conduct comprehensive patient assessments Provide support coordination within a recovery framework to include treatment and community supports with both formal and informal networks Ensure clients are supported in community environments, with engagement in community locations at a minimum of 75% of the service period What We’re Looking For Bachelor’s in Psychology, Social Work or other related field Valid registration with the relevant college/governing body in Ontario Knowledge of older adult population needs and concurrent mental health disorders 1 – 3 years’ experience in concurrent mental health and knowledge of the complexities within the senior population Valid driver’s license, business insurance and personal vehicle Hours of Work Permanent Full Time, 35 hours per week As part of our commitment to the health and safety of our patients, staff and community from COVID‑19, subject to any accommodation required by applicable human rights legislation, Homewood Health Centre requires that all staff have received all required doses of a COVID‑19 vaccine approved by Health Canada.
