Program Coordinator- Community InclusionJoin to apply for the Program Coordinator- Community Inclusion role at Pacific Coast Community Resources Inc.OverviewJOB SUMMARY: The Community Inclusion Program Coordinator will have the personality, ability and temperament necessary to interact with the people we support, their families and advocates, Community Inclusion employees and all others in a manner that fosters rapport and encourages open communication to ensure standards are met and quality services are provided. The Program Coordinator shall be responsible for the daily operation of their assigned programs and management of their Community Inclusion employees. The Coordinator is also expected to work and collaborate with colleagues as part of PCCR’s Home Share Coordinator network team.Key Duties & ResponsibilitiesAttend procurements on Community Inclusion programsCreate and set up Community-based programs for supported personsRecruitment, retention and management of Community Inclusion (C.I.) employeesAssist in program training, staff development and provide support and guidance to Community Inclusion employeesAssist with the development of Individualized Service Plan / Personal Service PlanSupport Home Share Providers and Community Inclusion workers in achieving goals and objectives as stated in ISP/PSPEnsure each person’s Personal Service Plan is regularly implemented, reviewed and updatedParticipate in developing the strategic direction of Community Inclusion programs at PCCRMonitor programs and ensure all regulatory guidelines and standards including CARF are metCoordinate crisis response and assist with problem-solving and respite; understand Risk Management and implement Behavioral Support Plan as part of Crisis Response for C.I.Available to answer calls after hours regarding Community Inclusion concerns/issuesManage program hours accordinglyEvaluate C.I. employees’ progress annually in writing; provide follow-up direction and discipline where necessaryAttend Home Share Coordinator’s meetings and participate in all other meetings as requiredEnsure all C.I. employees are logging their hours; all personnel, payroll and financial records are maintained as requiredComplete administrative tasks related to contracting with Community Inclusion programMeet with Directors / Home Share Manager and perform tasks as requestedQualificationsPost-Secondary DiplomaCertificate / Diploma in Management TrainingMinimum three years experience as a Community Inclusion worker or related field or at least two years of managerial experienceAbility to develop Community Inclusion resources and manage and monitor programsExcellent oral, written and interpersonal communication skillsStrong computer skills in Microsoft Windows; word processing and competency with Outlook email application and knowledge in Share VisionEvidence of practice of confidentialityCurrent First-Aid and CPR Level C CertificationAny qualification or skills required by the funding agencyBenefitsOn-the-job training including MANDT, JOHS, Diversity and Inclusion, Medication Safety, Confidentiality and Privacy, Trauma-Informed Practice, First Aid and CPR re-certificationCompetitive Salary $33.77- $38.77 per HourComprehensive Medical, Dental, and Vision care benefits packageAn inclusive, supportive work environment and community involvementA caring, fun, ambitious and supportive teamCompany InformationWe believe that a strong commitment to diversity & inclusion enables us to truly make services better for everyone.
