Job Description:
Position Summary:
•Provides social work services in the primary care clinical setting working with an interprofessional care team
•Participates in care plan development by assessing and planning to address social determinants of health needs that present barriers to recovery and adaptation;
•Utilizes a variety of treatment modalities for clients, caregivers and families including providing therapeutic clinical counseling services as appropriate,
•Cooperating and collaborating with other members of the Primary Care Team in the provision of quality, client-centered care;
•Facilitates access to a variety of community support services, making referrals as appropriate and providing input and feedback as required;
•Promotes and facilitates effective communication between the Primary Care Providers, Physician Specialists and the patient, caregivers/families;
•Develops support components to care plans that maximize the patient's inclusion, collaboration and communication with patients/caregivers/families, contracted support services, community agencies, and other sources of support for the patient;
•Promotes and facilitates patient independence;
•Reviews patient records to monitor and evaluate care/treatment
•Documents all patient encounters and interactions and communicates that information to the interdisciplinary team as required.
•Utilizes computerized systems to maintain patient records, including obtaining and entering patient demographics, histories and charts as required, in accordance with policies, procedures and professional practice standards
•Demonstrates personal initiative in maintaining professional currency and skills by participating in relevant educational activities, as required
•Provides best practice evidence and research based care by reviewing literature and attending workshops and education sessions as required
• If required, participate in research, program evaluation and data collection
• Familiarity with the social, economic and cultural conditions of the immediate community; experience in working with local service providers, community groups and networks, and an understanding of local issues and priorities are an asset.

Requirements:
• MSW degree and registered with the Ontario College of Social Workers and Social Service Workers
• Experience with Community/Primary Care
• Excellent crisis intervention skills
• Strong focus on psychosocial assessments and discharge planning
• Excellent assessment skills and familiarity with community services
• Strong ability and interest in linking patients to community resources
• Must be flexible and a strong team player
• Demonstrates excellent interpersonal and communication skills (verbal & written) and good judgement when relating to patients, families, co-workers and interdisciplinary teams
• Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
• Utilize protective equipment provided and identify and reports hazards
• Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
