We are looking for a dedicated Social Worker who is passionate about working with seniors in a long-term care setting. Working in collaboration with our internal and external partners to facilitate and provide counseling, identify resource availability, and education for Residents, family members and employees.

The Social Worker will work and think differently to create a safe and empowering environment that will support and respect the autonomy, independence, dignity and privacy of our Residents, while valuing learning and embracing innovative change using a person-centered and normalized living approach.

**Responsibilities**:

- On a referral basis and/or as necessary, offer adjustment counseling (education, one:one visits, support groups, etc.) to Residents and family members.
- Determine Resident, TSDM, etc. eligibility for financial or material assistance as provided by related organizations or governmental bodies.
- Consult with Complex Care Manager, RNs/RPNs/LPNs, Recreation Coordinator, etc. regarding Resident care and service plan development.
- Participate in Resident care conferences (as required).
- Provide crisis intervention and debriefing for Residents, family members and/or employees.
- Remain current and act as a resource for the Home with regards to legislative programs, changes and/or advances in social service related to complex care such as adult guardianship.

**Qualifications**:

- Bachelor’s Degree in Social Work, registered or eligible for registration with the British Columbia College of Social Workers.
- Expertise in psychosocial counseling, loss and grief counseling, abuse issues and end of service planning.
- Knowledge of community resources, financial management, conflict management, etc. that will be of benefit for Residents/TSDM and employees.
- Knowledge of altered cognitive function (particularly various dementias) and the normal ageing process.
- Excellent communication and interpersonal skills coupled with a high level of professionalism and confidentiality.
- Fluency or a strong understanding of Cantonese and Mandarin will be considered an asset.
- You have a genuine passion for caring for our residents.
- You can pass a Criminal Record Check for working with vulnerable adults.
- Provide proof of COVID-19 vaccinations.
