Patient Care Manager

Reporting to the Program Director, the Patient Care Manager is a member of the management team within the program/clinical area and is responsible for the coordination and direction of the all surgical ambulatory services which includes: C5- outpatient procedures services, endoscopy, cystoscopy, minor surgery, ophthalmology, breast health, fracture, plastics, and MSK clinic at all sites of Mackenzie Health. This includes the human and fiscal resources of the outpatient program. The Patient Care Manager collaborates with the Program Director, Medical Directors, Perioperative Manager, Educator, allied health members as well as other program stakeholders to ensure that high quality, effective and efficient care is provided. The Patient Care Manager is accountable for planning, implementing, and integrating plans that are in alignment with the organizations vision and strategic directions.
Primary Responsibilities:
Responsible for the management, operations, and organizational effectiveness of the department, including managing all aspects of human and financial resources collaborating with the team in developing, implementing and managing short and long-term strategic priorities and quality improvement initiatives.
Contributes to the organization and program vision, values and strategic direction that align with organizational priorities.
Supports and maintains the principles of shared governance, interdisciplinary and collaborative management, patient focused service, and continuous quality improvement.
Works collaboratively with the Program Director and other key interdisciplinary members to address operational issues that impact the delivery of service and the efficient use of resources.
Ensures the development and use of appropriate resources and supplies to measure and evaluate processes and outcomes.
Provides leadership in the development of new initiatives by creating an environment that encourages creativity, innovation, and critical thinking among the disciplinary team.
Participates in the development of the operating plan in consultation with stakeholders such as physicians, staff, support services and departments, consumers and community partners.
Acts as a coach, mentor and role model to team members and provides leadership to all staff by providing an environment where shared governance, empowerment and respect for diversity can grow.
Fosters effective employee and labor relations through knowledgeable interpretation and implementation of hospital policies and collective agreements.
Other duties as required, including participation in the manager on-call rotation
Education/Certifications/Licenses:
Completion of Baccalaureate Degree in Nursing or other regulated health professional field. Masters preferred.
Nurse Certificate of Competence from the College of Nurses of Ontario or another regulated Health Professional College
Certified Health Executive (CHE) with Canadian College of Health Leader’s preferred
Experience/Skills/Abilities:
Minimum of 2 -5 years clinical patient care management experience required
Ambulatory leadership and experience preferred.
Demonstrates knowledge in clinical practice.
Demonstrates excellence in interpersonal, communication, coaching, mentoring and team building skills that facilitate positive interactions among all members of the health care team.
Demonstrates a clear understanding of the application of the RHPA legislation.
Demonstrates superior understanding of systems, systems analysis techniques, value analysis process and continuous improvement principles and tools.

Demonstrates a strong patient and customer focused philosophy in all interactions.

Experience with guiding staff experiencing performance and/or attendance problems.
Ability to perform the essential duties of the job.
