Start Date: December 2025 – January 2026 Patient Care Coordinator – Full-Time Status: Full-Time Location: Toronto, Ontario (On-Site) About Perennial Perennial delivers integrated primary care and wellness services directly within retirement residences, combining on‑site and virtual care to meet seniors where they are. By proactively anticipating the needs of older adults, we improve quality of life, reduce unnecessary hospital visits, and ease the strain on the public healthcare system. Today, we are active in over 29 retirement residences across Ontario, Saskatchewan, Alberta, and Manitoba, with continued plans for growth and expansion. Our interdisciplinary team—including physicians, nurse practitioners, registered nurses, physiotherapists, occupational therapists, dietitians, and massage therapists—works together with one clear mission: to keep residents healthy, active, and comfortable in their own homes. About The Role We are seeking a Full‑Time Patient Care Coordinator to join our team in Toronto, Ontario, starting December 2025 or January 2026. In this role, you will serve as the primary point of contact for residents and families in Ontario and Alberta, providing day‑to‑day support and ensuring seamless communication between residents, their care providers, and our operations team. You’ll be the person residents and families turn to when they have questions, need help coordinating services, or require assistance navigating their care plans. This is a highly resident‑facing role where you will balance communication, coordination, and administrative support to ensure that residents receive timely, compassionate, and well‑organized care. Responsibilities Resident & Family Support : Act as the “go‑to” contact for residents and families in Ontario and Alberta, responding promptly to calls, emails, and texts. Care Coordination : Schedule and confirm appointments with nurse practitioners, physicians, and allied health providers. Coordinate referrals and follow‑ups with external specialists as required. Clinical Communication : Liaise directly with primary care providers, nurse practitioners, and operations staff to ensure clear, accurate, and timely communication about resident needs. Issue Resolution : Troubleshoot resident questions or concerns, escalating to clinical or operational leadership as needed. Program & Service Navigation : Guide residents and families through care plans, insurance submissions, and wellness program offerings. Operational Support : Partner with operations to ensure smooth day‑to‑day workflows across homes, including coordinating provider schedules and supporting logistics. Documentation : Maintain accurate and up‑to‑date records in electronic medical records (EMRs) and internal systems. Relationship Building : Foster trusted relationships with residents, families, and on‑site staff to support a consistent, positive care experience. Event & Engagement Support : Assist with resident wellness programming and on‑site events in Ontario, ensuring Perennial remains a trusted partner in each community. Qualifications Experience : 2+ years in a patient‑facing or healthcare coordination role (clinic, hospital, retirement, or wellness setting strongly preferred). Education : Diploma or degree in healthcare administration, medical office administration, social services, or related field. Skills : Excellent organizational skills, ability to multitask, and strong written and verbal communication skills. Interpersonal Strengths : Empathetic, approachable, and resident‑focused, with the ability to build trust and foster long‑term relationships. Technical Skills : Comfortable with EMRs, scheduling tools, and Microsoft Office/Google Suite. Flexibility : Ability to support residents and families across multiple provinces, with occasional travel to homes in Ontario if required. Languages : Additional languages are an asset given the diverse senior population. Why Join Perennial? High‑Impact Role: Be the trusted first point of contact for seniors and their families, directly shaping their care experience. Cross‑Provincial Impact: Support homes across Ontario and Alberta, helping ensure consistent, quality care no matter where residents are located. Growth Opportunities: Pathways to more senior resident experience or operations leadership roles as Perennial continues to expand. Collaborative Team: Work closely with a dynamic team of healthcare providers and operations staff committed to reimagining seniors’ care. Mission‑Driven Organization: Contribute to reducing unnecessary hospital visits and keeping seniors healthy and comfortable at home. Perennial is an equal opportunity, affirmative action employer. All qualified applicants will receive consideration for employment without regard to race, colour, religion, sex, sexual orientation, gender identity, national origin, protected veteran status, disability, or any other characteristic protected by law. If you are an individual with a disability and require a reasonable accommodation to complete any part of the application process, or are limited in the ability or unable to access or use this online application process and need an alternative method for applying, you may contact careteam@perennialcare.ca for assistance.
