We are currently looking to fill a Casual opportunity for a Resident/Client Care Coordinatorat Queen’s Park Care Centre located in New Westminster, BC.

Queens Park Care Centre (QPCC) is located in New Westminster and provides rehabilitation services, an adult day program and a long-term care community. QPCC has an outpatient rehab service and 76 specialized inpatient rehabilitation beds. These provide intensive rehab for different patient populations including Brain Injury, Spinal Cord Injury and Stroke rehab patients as well as lower leg amputees, complex neurological conditions and other muscular skeletal injuries. Queens Park is a small and friendly site with a close link to its community and a high dependency on it’s multidisciplinary team. Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Resident/Client Care Coordinator provides clinical supervision, guidance and support to nursing and nursing support staff and coordinates clinical team members within the assigned unit(s). The Resident/Client Care Coordinator has responsibility for the day-to-day operations of the assigned area(s) and supports/participates in change management, quality improvement, risk management, policy, program, research and/or regional activities.
Responsibilities

  1. Applies established criteria and prioritizes activities related to residents/clients and ensures individual care plans are developed and implemented to meet the ongoing needs of residents/clients, in collaboration with the other clinical team members and residents'/clients' families.
  2. Supervises nursing and nursing support staff, observes work and corrects where necessary, assigns and adjusts work assignments, schedules staff, monitors and recommends staff hours of work, including overtime. Coordinatesthe work of theclinicalteam overallon a day-to-day basis;monitors the quality of care and the safety of residents/clients and takes corrective action as appropriate.
  3. Supports qualityresident/client care and excellence in practice bypromoting interprofessional collaboration and team building,supporting staff to plan and carry out resident/client/family educationregarding chronic illness, palliative care, etc; acts as a mentor and resource to staff,role modeling best clinical practices,assessing clinical skills andcoaching staff and/or liaising with other health professional leaders to help them achieve established clinical performance goals.
  4. Participates in or leads theinvestigation of complaints by residents/clients/families; follows up on complaints and implements corrective action as appropriate. Collaborates with Quality Improvement according to established protocols.
  5. Initiates/leads/supports plan of careconferences and roundsfor the purpose of optimalresident/client care management according to best practice;disseminatesinformation to nursing/nursing supportstaff and other members of theclinical team.
  6. Identifies learning requirements of staff and, in conjunction with the Manager, Educatorand other members of the clinical team, provides input intothe development of learning objectives and may contribute to the achievement of the learning objectives; provides clinical support and guidance and evaluates the effectiveness of learning opportunities.
  7. Participates in the orientation of new staff and provides input into the development of a comprehensive orientation program, in collaboration with the Educator and other clinical staff.
  8. Participates in change management, quality improvement andrisk managementactivities by methods such as evaluating effect of changes in practice, generating recommendations for alternative approaches to practices, and conducting safety audits. Supports teaching/learning and implementsteaching/learning as assigned.
  9. Assists the Manager/Director by attending panel interviews, providing recommendations on the selection of new staff, providing feedback to the Manager on staff performance appraisals, and providing feedback on optimal use of budgetary resourcesincludingminor and capital equipment needs as requested.
  10. Promotes staff awareness and involvement in research/evaluation activities andprojectsand participates in research andprojectsas appropriate.
  11. Collaborates with internal and external stakeholders, participates on a variety of designated professional or operational committees at the unit, site, program and/or regional level.
  12. Participates in the review, development, implementation and evaluation of evidence-informed clinical decision support tools/policies.
  13. Performs other related duties as assigned.

Qualifications

Education and Experience

Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent combination of education, training and experience. Certification applicable to the designated clinical area.

  • Ability to role model, coach and mentor best practices inresident/client care.
  • Ability toidentify individual or group learningpriorities to meet identified program, organizational and learner needs, in collaboration with other clinical staff.
  • Ability to support implementation ofeducation based on changing practices e.g. legislation and technology.
  • Ability to provide input into budgets andminor and capital equipment.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to work independently and in an organized and self directed manner.
  • Ability to problem-solve and effectively deal with conflict situations.
  • Ability to operate related equipment including applicable software.
  • Physical ability to perform the duties of the position.
