We also believe that language, literacy, and poverty should not get in the way of receiving great care.Our team provides primary care, illness prevention and health promotion services and a range of community programs primarily to the residents of Dutton Dunwich and West Elgin municipalities. Our diabetes services are open to anyone and are offered at multiple locations in Elgin County. We strive for a healthy and supportive work environment valuing compassion, excellence, collaboration, accountability, respect, and equity.The Social Worker is an integral member of the interdisciplinary primary care team at the West Elgin Community Health Centre, supporting Primary Care Providers (PCPs) in delivering stepped care and short-term focused interventions. This role emphasizes brief, solution-focused counseling (typically 2–3 sessions), psychosocial assessments, and care coordination. Clients requiring longer-term or intensive therapy are referred to the Centre’s Psychotherapists.The position promotes trauma-informed, culturally safe, and client-centered care, and contributes to health promotion, education, and quality improvement initiatives within the Centre and community.Graduate degree in a relevant discipline from a recognized university.Registered and in good standing with the Ontario College of Social Workers and Social Service Workers.Three to five years’ counseling / therapy experience; preferably in a community health setting.Proficiency in current assessment, psychotherapy, and psychosocial counseling techniques.Excellent communication, collaboration, and interpersonal skills.Strong understanding of trauma-informed care, ACEs, and attachment theory.Knowledge of relevant legislation (e.g., Mental Health Act, Health Care Consent Act).Ability to manage complex clinical and psychosocial situations.Experience in program development, implementation, monitoring and evaluation.Ability to work independently and as part of a collaborative team.Proficiency in the use of computers, EMRs, and various software. #J-18808-Ljbffr
