Responsibilities include but are not limited to:Responsibilities include but are not limited to: Provides pre-admission psychosocial assessments as well as ongoing crisis intervention, discharge planning and counseling to patients and their families, including advanced care planning and end-of-life care, primarily with patients with responsive behaviours and mental health needs Provides support/consultation to other disciplines on the team re: psychosocial issues, particularly related to complex patient/family situations and responsive behaviours Acts as the team expert on discharge planning and patient flow, including reviewing referrals and liaising with VBM Acts as a linkage and communication conduit between the family and the care team Establishes community linkages and outreach with relevant agencies and other Baycrest programs and services, including VBM and BSO Participates in teaching and other student supervision activities for health professional students assigned to this unit Qualifications include but are not limited to: Masters of Social Work degree Current and valid registration with the Ontario College of Social Workers and Social Service Workers (‘OCSWSSW’) Experience in Mental Health preferred Experience with Responsive Behaviours preferred Experience in discharge planning and system navigation preferred, including knowledge of VBM and BSO Experience working in a healthcare setting or mental healthcare setting preferred Excellent interpersonal, communication and problem-solving skills Basic computer skills – Meditech, Microsoft Word and Excel, RM&R preferred Additional Benefits: Vacation Entitlement on pro-rated basis Opportunity to enroll in the extended health and dental benefit plan Opportunity to enroll in the Healthcare of Ontario Pension Plan (HOOPP) Access to 24/7 Employee Assistance Program
