**SOCIAL WORKER MSW 1, PART TIME**

35 Bi-weekly Hours - Day, No Weekends

OPSEU 595 Hourly Pay Rate: $38.73 - $49.36

The Social Worker is an integrated member of the Ambulatory Services teams, providing support to the Medical Specialty Clinics (Geriatric Assessment Clinic) and the Neuropsychology and Cognitive Health Program (Learning the Ropes for Living with MCI). The Social Worker in this position demonstrates a high degree of practice excellence, and functions as a clinical resource for the interprofessional team as well as the patient and family. This position reports directly to the Clinical Manager-Ambulatory Services and indirectly to the Program Director, Neuropsychology and Cognitive Health and the Manager & Interprofessional Practice Chief for Social Work.

**Responsibilities include but are not limited to**:

- Provide full range of clinical social work services (psychosocial assessments, case management, co-ordinate patient care conferences, clinical triage & intake, brief individual and family counseling, resource navigation/referral and psycho-education) to clinic patients and their families, as appropriate for the clinic
- Facilitate a manualized psychosocial support group focused on discovering approaches for effectively living with a family member experiencing mild cognitive impairment
- Support patients and family with advanced care planning, decision making and clarifying POA/SDM status, as appropriate for the clinic
- Maintain accurate, up to date clinical documentation and liaise with the interprofessional teams as required
- Participate in teaching and other student supervision activities for health professional students

**Qualifications include but are not limited to**:

- Masters of Social Work degree required
- Current and valid registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) in good standing required
- Experience working with adults and seniors living with mild cognitive impairment, dementia, mental health issues, and families/caregivers across clinical settings and able to provide the spectrum of supports (i.e. prevention, crisis, self-management, treatment, etc.) and a comprehensive understanding of the diverse needs of patients & caregivers
- Experience with identifying and using a range of clinical assessment tools, e.g., suicide risk assessment, depression scales
- Experience running support groups
- Expert knowledge of Ontario LTC, hospital & community care environments, pertinent health services legislation (i.e. privacy, LTCH Act, Hospital Act, MAID), clinical ethics, Ontario Senior Support Services
- Excellent interpersonal communication, solution oriented & problem solving skills
- Proficient computer skills & recent experience with Outlook, Meditech, Word, Excel, Resource Matching and Referral (RM&R), ConnectingOntario required and OTN/ZOOM Virtual Care delivery preferred

**Total Compensation**:
This is an unionized position with OPSEU 595.
