As a Speech-Language Pathologist (Permanent, full-time) you areresponsible for the provision of speech-language pathology services to OntarioHealth atHome patients to address and improve communication and/or swallowingdisorder.

Whether you work in our office, in the community, or a health carefacility – you will play a lead role in providing connected, accessible,patient-centred care – and be supported by our collaborative team that includesover 9,000 regulated health care and other professionals.

What will you do?

· Respond to patients’ referrals from Care Coordinators to properlyadminister speech-language pathology services.

· Assesspatients’ communication and/or swallowing abilities, using a variety ofstandardized and non-standardized assessment tools, to determine clienttreatment needs and therapy goals.

· Develop and implement treatment program and/or compensatorystrategies/techniques to address established therapy goals as they relate tocommunication, language skills, and/or swallowing function.

· Oversee patients’ progress and adjust treatment programs whenneeded, to ensure therapy goals are met.

· Educate and train patients, their families, caregivers, and/orSDMs, on how to progress with the treatment plan.

· Link patients with other in-house services or outsideorganizations if needed.

· Balance patients’ needs and choices with available resources,ensuring patients’ values and preferences are respected.

· Accurately document and chart patients’ assessment results, goals,treatment plans, and interventions.

What must youhave?

  • RegisteredSpeech-Language Pathologist in good standing with the College ofAudiologists and Speech-Language Pathologists of Ontario (CASLPO).
  • In-depthknowledge of speech-language pathology services and other communityresources, treatment centres, and organizations.
  • Goodunderstanding of the roles of other health care professionals.
  • Relevantspeech-language pathology experience in a community/health careenvironment.
  • Comprehensiveand up-to-date knowledge of speech-language pathology tools, processes,equipment, and assistive technology.
  • Practicalknowledge and understanding of relevant legislation (e.g. regarding theprovision of health care services, privacy, health, and safety, etc.).
  • Comprehensiveknowledge of the standards of practice and professional guidelines setforth by CASLPO.
  • Abilityto prioritize professional duties and manage multiple patients, andefficiently organize workload.
  • Stronginterpersonal and collaboration skills to work with diverse patientgroups, Care Coordinators, family members/caregivers/Substitute DecisionMakers, health care professionals, community organizations and serviceproviders.
  • Effectivelistening, observation, and facilitation skills.
  • Abilityto communicate information effectively through a variety of meansincluding meetings, reports, letters, and presentations.
  • Workingknowledge of computer software (email, internet) and Microsoft Officeapplications (Word, Excel).
  • Musthave valid driver’s license and access to a vehicle, as regular travelwithin the Cochrane district will be required. Travel to other areaswithin the North East geography may also be required from time to time, asthe business needs require.
  • Advanced oraland written proficiency in English is essential; French is an asset.

· Case managementexperience or recent related community experience

· Abilityto speak French or another second language
