Job Summary:
The Supportive Housing Supervisor is responsible supporting the daily operations of supportive housing programs. This role includes supervising staff, assisting with coordination of services for consumers, ensuring compliance with regulations, and fostering a positive living environment that promotes independence and well-being.

This is a full time position. Hours of work will be 11:00am – 7:00pm from Monday to Friday and alternating weekends. Hours would include a thirty (30) minute paid meal break each day. However, as Nucleus provides services on a 24/7 basis, flexibility with your scheduled hours of work is expected to resolve issues that occur outside of regularly scheduled hours. This will include working evenings, weekends, and holidays and on call as may be required, depending on business requirements.

  • Supervise PSWs: Lead, mentor, and support the PSW team in delivering high-quality personal care services to residents.
  • Training & Development: Provide ongoing training and professional development opportunities for PSWs to improve skills and knowledge in line with industry standards and regulatory requirements.
  • Performance : Conduct regular audits, provide feedback, support conflict resolutions, daily scheduling coordination conflicts, and address performance issues as needed.
  • Team Building: Foster a positive team culture, ensuring effective communication, collaboration, and support among team members.

2. Resident Care & Service Coordination

  • Ensure Quality Care: Monitor and ensure the PSW team delivers person-centered care, respecting the dignity, privacy, and rights of residents.
  • Develop Care Plans: Work with case managers, nurses, and other healthcare professionals to develop, implement, and review personalized care plans for each resident.
  • Resident Safety & Wellbeing: Ensure that residents' needs are being met, and that any changes in health or behavior are reported and addressed promptly.
  • Monitor Client Progress: Regularly review client care progress and adjust care plans as needed in collaboration with other healthcare professionals.
  • Support Emotional & Social Needs: Promote the mental and emotional well-being of residents, encouraging social engagement and community participation.

Policy, Compliance & Documentation

  • Ensure Regulatory Compliance: Ensure all services provided by PSWs comply with the applicable laws, regulations, and organizational policies (e.g., Ministry of Health and Long-Term Care, tenant rights).
  • Documentation & Reporting: Oversee the accurate documentation of client care activities, incidents, and any relevant updates, ensuring that records are up-to-date and compliant with confidentiality standards.
  • Incident Management: Investigate incidents or complaints related to PSWs, report findings, and implement corrective actions as necessary.
  • Health & Safety: Ensure staff adhere to health and safety protocols, including proper lifting techniques, infection control practices, and emergency procedures.
  • Multidisciplinary Collaboration: Work closely with other team members, including housing staff, mental health professionals, addiction specialists, social workers, and healthcare providers, to ensure integrated care delivery.
  • Family & Community Liaison: Serve as a liaison between residents, their families, and external service providers, ensuring communication is clear and that needs are met.
  • Referral & Resource Navigation: Help connect residents to additional community resources and services such as healthcare, social services, and employment training.

Qualifications:

  • Degree or diploma in social work, allied health, psychology, human services, or a related field.
  • Minimum 3 years of experience in supportive housing, social services, or a related field.
  • Strong understanding of housing-first principles and trauma-informed care.
  • Excellent communication, leadership, and interpersonal skills.
  • Ability to work collaboratively in a diverse team environment.
  • Knowledge of community resources and support services.
  • Flexibility to work at various sites and adjust to weekend and rotational schedules.
  • Valid driver's license.
  • Proficient in Microsoft Word and comfortable with technology.
