Job Overview

We are seeking a compassionate and skilled Intensive Case Manager to join our team at Homes First Society. As a member of our case management team, you will play a critical role in supporting individuals experiencing homelessness or housing instability.

The successful candidate will have a strong background in social work, case management, or a related field, with excellent communication and interpersonal skills. You will work closely with clients to assess their needs, develop individualized plans, and provide ongoing support to help them achieve stable housing and optimal health and mental health outcomes.

This is a full-time, contract position with a competitive hourly rate. The successful candidate will be required to work Monday to Friday, 10am-6pm, with one late day per week. A valid driver's license and reliable transportation are essential for this role.

Key Responsibilities:

  • Provide comprehensive support services to individuals experiencing homelessness or housing instability
  • Develop and implement individualized case management plans and crisis plans
  • Connect clients to community resources and support services
  • Work collaboratively with other case managers to develop and provide referrals for specialized supports
  • Maintain accurate records and data collection

Qualifications and Requirements:

  • Degree in social work, case management, or a related field
  • Minimum one year experience in a related field
  • Excellent communication and interpersonal skills
  • Ability to compile data and prepare statistical reports
  • Certificate of Completion of Homelessness Learning Hub's Harm Reduction for the Homelessness Sector considered an asset
