Pay Band: $27.54 - $33.16 /hr.

Job Opportunity
The Practice Facilitator works closely with the Team Lead for the Community Response Team (CRT) and NESI Program at the Somerset West Community Health Centre. The Practice Facilitator supports the Community Response Team, comprised of Community Workers (CWs), as they provide community response services in a designated area. Although this support provides opportunities to reflect and learn, it also involves coaching and working alongside Community workers.
Position Description
The Practice Facilitator will be responsible for supporting the Community Response Team (CRT) at Somerset West Community Health Centre through ongoing training and regular reflective practice sessions focused on topics such as boundaries and self-care and workplace norms. In addition, they will also be responsible for working alongside CRT delivering outreach during evening and weekend hours. This will include providing real-time support and development to CRT in providing service delivery.
Key Competencies
- Relevant combination of education AND/OR knowledge AND/OR experience equal to that of a Bachelor of Social Work (BSW) degree, OR a BSW
- Lived experience of drug use an asset/preferred
- Knowledge and experience with a harm reduction model of service delivery
- Drug, sex and sex worker positive
- Understanding of HIV and Hepatitis transmission
- Knowledge of related community resources
- Understanding of homelessness, mental health and substance use
- Experience with group facilitation, in particular bridging diverse and divergent opinions
- Experience developing, delivering and evaluating training materials
- Experience supporting and supervising people who use drugs an asset
- 3-5 Years’ experience providing direct service to people who use drugs from a Harm Reduction lens.
- Strong knowledge and ability in navigating issues related to clinical boundaries, professional ethics, confidentiality and relevant legislation including but not limited to PHIPPA and OHSA
- Commitment to working within a Health Equity, Person-Centered and Anti-Oppressive Framework
- Knowledge and experience working with computers
- Experience in Case Management an asset.
- Experience with Job Skills programs an asset.

- Fluency in English
- French and other languages an asset/preferred
Physical Requirements:
- Ability to lift and carry items approximately 25-30 lbs. daily through extended foot outreach.
- This position will require the ability to walk approximately 10-15 km over the course of a shift, including on uneven ground and with limited visibility.
- Ability to safely collect used inhalation or injection equipment which carries the risk of potential needle stick injury and exposure to blood borne pathogens.
Mental/Emotional Requirements:
- Ability to make rapid decisions about safety and risk and a high degree of clinical judgment with unknown factors.
- Ability to assess safety in engaging with people who may be in crisis or in rapidly shifting stages of escalation.
- Ability to work in environments that are fast paced, loud in volume, with people who may be in crisis.
- Ability to work within a team of people from diverse backgrounds and life experiences.
- This position will require the ability to assess the safety of entering unknown locations (usually in public), including at night to engage with people who may be in crisis.
- Regular exposure to significant psychological and emotional stressors, vicarious trauma, and potential risk of violence.
Other Job Requirements
- Needle exchange and safer inhalation training
- Valid CPR
- Ability to travel offsite on a weekly basis
- Valid Driver's License and ability to drive Centre vehicles
- Obtain Vulnerable Sector Screen Police Record Check for Executive Director review and sign-off. NOT required to be clear check.
