The Downtown East Toronto Ontario Health Team (DET OHT) is a group of 43 health and social service provider organizations (Team Members) who have deep roots in the community and is committed to creating an integrated system of care. The vision of the DET OHT is to provide our community with seamless access to inclusive, high quality, and responsive care through one connected system of diverse providers and community members who enthusiastically embody holistic approaches to improving population and individual health and well-being.

In Fall of 2022, Team Members have approved the Strategic Plan, which lays out the strategic priorities for 2023-2025. The DET OHT is in its first year of operations and rapidly developing its new, integrated approach to health care. The DET OHT’s work is centered on key principles to bring this vision forward including: Client and Family Centered Care; Health Equity; Diversity, Equity and Inclusion; Transparent Collaboration; Quality Improvement; and Responsive Leadership.

The work of the Downtown East Toronto OHT includes focused initiatives and planning to improve care for our priority populations (People Experiencing Homelessness and Mental Health and Addictions) ensuring community members are meaningfully part of all of our work, co-designing change ideas, developing digital solutions, managing privacy and information management, strengthening governance, supporting performance and quality improvement, and developing a strategy for the OHT.

about the role:

Working under the supervision and guidance of the DET OHT Manager of Operations and Population Health, the Summer Project Assistant will work with DET OHT’s health and social service provider organizations, the Primary Care Attachment Steering Committee and the DET OHT Operations Team to support the DET OHT in providing expanded comprehensive wrap-around primary care services in a target approach to connect DET residents who are lacking a primary care provider and are facing significant financial challenges, including but not limited to those experiencing housing instability, food insecurity, and other forms of marginalization, as well as those hindered from accessing comprehensive team-based care.

With supervision and guidance from the DET OHT Manager of Operations and Population Health, the Summer Project Assistant will provide research and administrative assistance to support the successful launch and implementation of the aforementioned initiative, in addition to general support to the DETOHT. This position is ideal for individuals interested in healthcare system transformation and community health programming.

Depending on funding and implementation constraints and timelines, some of the deliverables that the Summer Project Assistant will support may include:

  • Updated visuals of proposed service delivery pathway and providers
  • A comprehensive database of relevant health and social services to support the target population.
  • Communication products detailing available services.
  • Functional and user-friendly tools to enhance service delivery and client intake processes.
  • Regular updates and maintenance of project materials and resources.

The Summer Project Assistant will also be tasked with general administrative duties such as cleaning up slide decks, updating databases and websites, taking notes, and transcribing. As such, the successful candidate will be expected to participate in the ongoing business of the DET OHT (e.g., attending meetings, participating in committee work, writing reports) to provide support to colleagues and the project team as required.

The responsibilities and skills of the DET OHT Summer Project Assistant may include, but are not limited to:

Research Skills

  • Review and synthesize peer-reviewed and grey literature, and analyze internal documents and databases related to project needs.
  • Conduct a thorough environmental scan to assess primary care needs for the target population.
  • Developing data collection methods, including survey design, through appropriate platforms i.e. Survey Monkey
  • If needed, support key informant interviews, through development of interview guide and synthesis and analysis of key findings.
  • Providing summary of evidence-based findings to the Supervisor, organized in relevant population health themes.
  • Cleaning up and analyze data throughout project.
  • Developing key themes and findings, and compare survey data with evidence review with close supervision from the Operations Manager

Project Design and Implementation

  • Develop and maintain a database that consolidates health and social services information.
  • Create visual summaries of services to support attachment to and access to comprehensive primary care for the target population.
  • Design and create tools to facilitate the successful implementation of health services, such as a screening/intake tool and a navigation cheat sheet.

Project Management and Administration

  • Writing meeting minutes and notes during meetings; transcribing meetings if needed
  • Updating internal databases as required
  • Developing project tools as required i.e. screening, intake.
  • Writing corporate emails as needed
  • Sending email reminders as necessary
  • Scheduling meetings including via Zoom and/or phone as necessary.
  • Monitoring data collection and informing Supervisor of any issues as they arise.

Community Member Engagement

  • Engage Community Member in co-design.

Additional Duties

  • Performs other duties as required.

To thrive in this role, you’ll need

education and experience:

  • At least 3 years of undergraduate experience, preferably in Public Health, Community Health Sciences, Social Work, Psychology and/or equivalent
  • Experience working with priority populations is an asset.
  • Proven interpersonal and organizational skills in a professional setting.
  • Diligent and self-motivated with a high proficiency in written and verbal communication.

skills and abilities:

  • Understanding of community health priorities is an asset.
  • Must have basic research skills including ability to review the evidence-base, including retrieving peer-reviewed journal articles.
  • Ability to design research tools including but not limited to surveys or key informant interview guides.
  • Excellent organizational and administrative skills.
  • Proven ability to meet tight deadlines and achieve results in a timely manner.
  • Strong inter-personal skills and proven ability to build and maintain positive relationships with staff, peers and stakeholders.
  • Strong problem-solving skills and ability to recognize when to appropriately escalate.
  • Proficient in data management and visualization tools including but not limited to advanced use of PowerPoint. Ability to work independently and as part of a team.
  • Proficient computer skills including Zoom and Survey Monkey.
  • Excellent English language skills – both oral and written.

  • This position requires working in a fast-paced environment meeting the needs of multiple stakeholders. Time management, multi-tasking, initiative, organization and prioritizing are essential. Work is mainly performed in an office environment. Occasional work outside of normal working hours may be required, in order to meet with clients, vendors and partners.
