The Peritoneal Dialysis Clinical Resource Nurse will work in both the hospital and the community to support home dialysis patients. They will be able to autonomously assess, educate and train patients on home therapies, ensuring that they have the necessary knowledge and skills to perform dialysis independently within their home. They will be responsible to managing patient flow in the community, and supervising nursing assignments. They will provide support to patients in the clinic setting as well as during home visits. The nurse will also participate in on-call for the program. This individual will closely liaise with a multidisciplinary team. The Clinical Resource Nurse working in the Peritoneal Dialysis unit will utilize their expert knowledge to support the Peritoneal Dialysis Nursing Team in the creation, progress, and modification of Patient Care Plans and Goals. The dynamic planning of Peritoneal Dialysis resources and coordination with internal and external stakeholders is a pivotal aspect of this role.

  • Provides expert clinical direction and acts as a resource to support patients and staff within Renal Dialysis as it pertains to Peritoneal Dialysis
  • Coordinates and supports excellent care for those patients being cared for in the Home Dialysis Assistance Program in the community.
  • Supervises nursing flow and assignments within the department.
  • Assists with Peritoneal Dialysis patient assessment, education, training, and direct care including Body Access as needed including support at patient’s homes.
  • Ensures all patients are assessed and trained in a timely manner for Peritoneal Dialysis.
  • Takes the lead in working with other members of the multi-disciplinary care team regarding patient related issues facilitating resolution of barriers and access issues.
  • Ongoing assessments of Peritoneal Dialysis activities during the day to ensure training needs of the patient and staff assignments are aligned.
  • Coordinates/facilitates appointments of patients including home visits throughout the community.
  • Applies relevant research to clinical practice using critical thinking and decision making; utilizes research/evidence based information to recommend nursing interventions.
  • Organizes and leads weekly meetings with the Peritoneal Dialysis Team, Kidney Care Clinic Team, and CRN to identify patient navigation issues related to independent dialysis and participates in weekly multidisciplinary rounds and all Best Start patient reviews.
  • Develops and implements clinical protocols, care paths, algorithms and patient education materials.
  • Develops, implements and tracks processes to improve the performance metrics for Peritoneal Dialysis as described by the ORN.
  • Provides leadership and oversight in the development, implementation and maintenance of data collection tools that provide statistical information on Peritoneal Dialysis in collaboration with the CRN and Decision Support staff.
  • Participates in Quality initiatives as it pertains to Peritoneal Dialysis.
  • Tracks and monitors infection control issues including peritonitis rates and follows-up with MD, Infection Control personnel to improve outcomes
  • Facilitates diagnostic interventions, consults, booking of PD catheters and the resolution of access and wait time issues in collaboration with the CRN and Nephrologists.
  • Is the single point of contact for facilitating the assessment of and coordination of all patient movement across the renal program including repatriation requests from other CKD Programs; patient transfers between the Regional Hub and Burlington campus; and patient flow from KCC to in-centre and/or home.
    • Current registration in good standing with the College of Nurses of Ontario required.
    • Minimum of 5 years’ experience in Peritoneal Dialysis
    • Minimum of 3 years’ Body Access experience required.
    • Experience leading a community program.
    • Current patient training proficiency in Peritoneal Dialysis.
    • Knowledge of the principles of adult education and experience in implementing them within a care plan for patients.
    • Experience in developing/implementing patient algorithms, care paths.
    • Proficiency in using Microsoft office including Word, Outlook, Power Point and Excel.
    • Demonstrated nursing leadership skills including change and conflict resolution skills.
    • Ability to direct staff in critical and complex situations to ensure optimal management of patient care needs.
    • Ability to work collaboratively with peers, PCM, PPC, MD´s and other multidisciplinary professionals.
    • Excellent teaching, interpersonal and public relation skills.
    • Demonstrates being patient and consumer focused; makes a commitment to meet or exceed the needs of all internal and external customers
    • Demonstrates fiscal responsibility.
    • Must have access to a vehicle for the purpose of transportation throughout the community
    • Must be physically able to perform the essential duties of the position.
    • Satisfactory employment and attendance record required.

Salary: $43.25 to $60.15 per hour
