Care Coordinator

Are you a registered nurse, physiotherapist, occupational therapist, social worker, dietitian, or speech language pathologist looking for a rewarding career that prioritizes patient care and professional growth?
As a Care Coordinator, you will play a lead role in providing connected, accessible, patient-centred care, working collaboratively with patients, caregivers, and healthcare practitioners to assess care needs, determine eligibility, and develop individual care plans.
**Key Responsibilities:** * Assess patient care needs and determine eligibility for services * Develop individual care and service plans in collaboration with patients and their families * Link patients with service providers and coordinate care plan delivery * Establish a helping relationship with patients and their families, respecting their values and preferences * Balance patient needs and choices with available resources **Requirements:** * Membership in good standing with the applicable regulatory body * University degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work * 2+ years of recent experience in community health or a related field * Knowledge of the healthcare delivery system and community resources * Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills * Effective time management, prioritization, and organizational skills * Ability to accurately complete required documentation, reports, and forms * Valid driver's license and access to a reliable vehicle * Proficient in a Windows environment **Preferred Qualifications:** * Experience working with diverse patient groups * Case management experience or recent related community experience * Ability to speak French or another second language **What We Offer:** * Attractive comprehensive compensation packages and benefits * Valuable development opportunities * Membership in a world-class defined benefit pension plan **About Us:** We are Ontario Health atHome, a healthcare organization committed to delivering responsive, accessible, integrated, patient-centred care.
