Key Responsibilities- Patient Assessment and Care Planning: Collaborate with patients and their families to assess care needs, determine eligibility for services, and develop individual care and service plans.
- Service Coordination: Link patients with service providers, coordinate and monitor care plan delivery, and establish a helping relationship with patients and their families.
- Resource Management: Balance patient needs and choices with available resources, ensuring patients' values and preferences are respected.
- Professional Qualifications: Membership in good standing with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.).
- Education and Experience: University degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work, and 2+ years of recent experience in community health or a related field.
- Skills and Abilities: Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills, as well as effective time management, prioritization, and organizational skills.
- Additional Requirements: A valid driver's license, access to a reliable vehicle, and proficiency in a Windows environment.
