We are seeking a highly skilled Registered Nurse/Registered Psychiatric Nurse to join our team at Fraser Health Authority. As a Gerontological Care Specialist, you will play a vital role in providing person-centered care to our residents and clients.
Key Responsibilities- Collaborative Care Planning: Work collaboratively with the interdisciplinary team to identify current or potential care concerns and strengths, plan interventions, and evaluate outcomes.
- Direct Nursing Care: Provide person-centered, direct nursing care, including wound care, dementia care, and palliative care, and develop care priorities in accordance with established policies and procedures.
- Assessment and Documentation: Collect information from various sources using skills of observation, communication, and physical assessment to enable the provision of quality care, and document relevant observations, assessments, and interventions.
- Communication and Teamwork: Communicate resident/client assessments and changes to physicians, resident/client care coordinators, and the interdisciplinary team, and participate in care planning to support decision making and person-centered care.
- Advocacy and Leadership: Act as an advocate to protect and promote residents'/client's rights to autonomy, respect, privacy, dignity, diversity, and access to information, and provide leadership and clinical guidance to the team.
- Education and Quality Improvement: Participate in the orientation and ongoing education of students and others, including LPNs, HCAs, and other members of the interdisciplinary team, and participate in quality improvement activities by gathering information and providing input into proposed policies and revisions of current policies.
- Graduate of an approved School of Nursing or Psychiatric Nursing.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year's recent, related gerontological nursing experience or an equivalent combination of education, training, and experience.
- Current Level 1 First Aid certificate.
