Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Halton Healthcare. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and families in our inpatient and outpatient settings.
Job Summary:The Nurse Practitioner will be responsible for providing comprehensive health assessments, symptom management, and promoting compassionate care of the terminally ill and their families. You will work collaboratively with physicians and the interdisciplinary team to develop and implement plans of care, provide family counseling, and staff support.
Key Responsibilities:- Provide comprehensive health assessments and develop individualized plans of care
- Manage symptoms and provide palliative care to patients and their families
- Collaborate with physicians and the interdisciplinary team to coordinate care
- Provide education and support to patients and families
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Current registration in the Extended Class (NP-Adult) with the College of Nurses of Ontario
- Completion of a Master Degree in Nursing
- Minimum of 3 years nursing experience in palliative care
- Exceptional knowledge in palliative care and pain & symptom management
- Formal training and education in Palliative Care
