Job Title: Nurse Practitioner

Halton Healthcare is seeking a highly skilled and compassionate Nurse Practitioner to join our team in the Rehabilitation and Geriatrics department. As a key member of our interdisciplinary team, you will provide high-quality patient care, working collaboratively with physicians and other healthcare professionals to deliver exceptional patient outcomes.

Job Summary:

The successful candidate will possess a Master's degree in Nursing and be registered in the Extended Class (NP-Adult) with the College of Nurses of Ontario. With a minimum of 3 years of experience in palliative care, you will have exceptional knowledge in pain and symptom management, as well as formal training and education in Palliative Care. Your expertise in areas such as end-of-life care, complex medical needs, and working with diverse communities will be highly valued.

Key Responsibilities:
  • Provide comprehensive health assessments, symptom management, and develop and implement plans of care for palliative care patients.
  • Collaborate with the interdisciplinary team to deliver high-quality patient care, including family counseling and staff support.
  • Utilize a broad range of therapeutic modalities to promote compassionate care of the terminally ill and their families.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care transitions.
  • Stay up-to-date with the latest evidence-based practices and guidelines in palliative care, incorporating this knowledge into your practice.
Requirements:
  • Master's degree in Nursing.
  • Registration in the Extended Class (NP-Adult) with the College of Nurses of Ontario.
  • Minimum 3 years of experience in palliative care.
  • Exceptional knowledge in pain and symptom management.
  • Formal training and education in Palliative Care.
  • Eligibility for cross-appointment to an academic institution (an asset).
