Job Title: Registered Nurse/Registered Psychiatric Nurse We are seeking a skilled Registered Nurse/Registered Psychiatric Nurse to provide person-centered care to diverse populations in a rehabilitation setting. Job Description: The Registered Nurse/Registered Psychiatric Nurse will work collaboratively with the interdisciplinary team to provide direct nursing care, develop care plans, and evaluate outcomes. The successful candidate will have a strong understanding of nursing theory and evidence-informed practice, as well as excellent communication and leadership skills. Responsibilities: • Collaborate with the interdisciplinary team to identify current or potential care concerns and strengths • Provide person-centered, direct nursing care, including wound care, dementia care, and palliative care • Collect information from various sources to enable the provision of quality care • Document relevant observations, assessments, and interventions • Communicate resident/client assessments and changes to the interdisciplinary team • Act as an advocate to protect and promote residents'/client's rights • Participate in quality improvement activities and provide feedback on clinical pathways and protocols Qualifications: • Graduate of an approved School of Nursing or Psychiatric Nursing • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM) • One (1) year's recent, related gerontological nursing experience or an equivalent combination of education, training, and experience • Current Level 1 First Aid certificate Skills and Abilities: • Knowledge of nursing theory and evidence-informed practice • Ability to apply clinical knowledge in practice • Ability to communicate effectively both verbally and in writing • Ability to provide leadership and clinical guidance • Ability to work independently and as a member of the interdisciplinary team About the Company: Fraser Health is a leading healthcare provider in Metro Vancouver and the Fraser Valley, serving over two million people.
