About the Role

We are seeking a skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing patient care needs, determining eligibility for services, and developing individual care and service plans.

Key Responsibilities
  • Collaborate with patients and their families to assess care needs and develop individual care plans
  • Link patients with service providers and coordinate care plan delivery
  • Establish a helping relationship with patients and their families, balancing their needs and choices with available resources
  • Ensure patients' values and preferences are respected
Requirements
  • Membership in good standing with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario)
  • 2+ years of recent experience in community health or a related field
  • Knowledge of the healthcare delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
  • Effective time management, prioritization, and organizational skills
  • Ability to accurately complete required documentation, reports, and forms
  • Valid driver's license and access to a reliable vehicle
  • Proficient in a Windows environment
Preferred Qualifications
  • Experience working with diverse patient groups (e.g., multicultural, homeless, palliative, acquired brain injury)
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
