About the Role

The Registered Nurse is a key member of our team, responsible for providing client-centered, evidence-based care to individuals and families in their homes and community. As a Visiting Nurse, you will work independently to assess, plan, implement, and evaluate care plans to meet the unique needs of each client.

Key Responsibilities
  • Manage and coordinate care, evaluating health outcomes and educating clients and families to meet health goals
  • Provide all levels of nursing care, from stable clients with predictable outcomes to those with acute, complex, and rapidly changing health needs
  • Develop and implement care plans, incorporating discharge planning and client-centered care
  • Perform technical nursing skills at a safe level, adhering to nursing principles and VON policies and procedures
  • Document nursing processes clearly, concisely, accurately, and appropriately, according to regulatory and VON standards
  • Collaborate with other healthcare professionals and community organizations to promote nursing best practices
  • Participate in quality improvement initiatives and maintain competence through continuing education and self-reflection
Requirements
  • Current registration as a Registered Nurse in the province of practice
  • Canadian Nurses Association certification in Community Health Nursing an asset
  • Clinical practice experience and experience in community health an asset
  • Strong written and verbal communication skills, with ability to engage with clients, staff, and the public
  • Ability to work independently, problem-solve, and adapt nursing interventions as needed
  • Capacity to adhere to strict confidentiality and maintain professional boundaries
