**Job Summary:** Seeking a Nurse Practitioner to provide geriatric primary health care and supportive care to clients and families in the community environment, with a focus on dementia care.

Reporting directly to the Director of LTC & CSS, the Nurse Practitioner is responsible for providing comprehensive geriatric assessments and implementing strategies to reduce falls, prevent delirium, and maintain optimal chronic disease management in the home. The successful candidate will be a member of the multidisciplinary team for the Home Dementia Care Program, a pilot project designed to provide enhanced support to individuals living in the community with moderate to late stage dementia.

The Nurse Practitioner will function in the roles of practitioner, consultant, educator, leader, and researcher in close affiliation with their local geriatric team. Duties include:

  • Integrating advanced knowledge and experience in delivering safe, effective, quality care to geriatric clients with dementia in a community, home-based setting;
  • Advocating within the health care delivery system on behalf of clients with dementia in primary care;
  • Liaising with the health care delivery system and community partners to optimize geriatric dementia care;
  • Assisting in the development, promotion, and implementation of best practice in geriatric primary care for dementia;
  • Interpreting and communicating results of investigations to clients, families, and primary physicians when appropriate;
  • Practicing prescriptive authority within the scope of practice for the Nurse Practitioner;
  • Practicing with autonomy and collaboration to perform other related duties as required.

The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety, and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks, and implementing improvements.

Required qualifications:

  • Graduation from a nurse practitioner program approved by Central Health;
  • Current registration as a nurse practitioner with the Association of Registered Nurses of Newfoundland and Labrador;
  • Current BLS for health care providers from the Heart and Stroke Foundation;
  • Gentle Persuasive Approach or willingness to complete next available course;
  • CNA certification in Gerontology or willingness to complete next available course;
  • Advanced and current knowledge in comprehensive geriatric assessments, dementia assessments, and proficient knowledge in differential diagnosis of dementia, delirium, and depression;
  • Possess strong abilities in the areas of collaboration, clinical reasoning, professional development, interpersonal effectiveness, conflict management, and teamwork;
  • Possess strong communication, problem-solving, decision-making, and teaching skills;
  • Demonstrated progressive clinical leadership skills;
  • Demonstrated organizational and time management skills;
  • Demonstrated ability to function autonomously in an advanced practice role;
  • Demonstrated ability to support and participate in research to improve practice and dementia care outcomes;
  • Demonstrated skills in computer, electronic charting, and database management;
  • Consistent demonstration of the core values of NL Health Services: Innovation, Compassion, Accountability, Respect, and Excellence (I CARE);
  • Use of a private vehicle is required to visit clients in their home setting;
  • A satisfactory record of work performance and attendance is required for this position;
  • A satisfactory Certificate of Conduct from the RCMP or local police authority.

Preferred qualifications:

  • Demonstrated skill in using health care-based computer systems, CRMS, and Meditech.

Hours of work: 75 hours bi-weekly (Shift work may be required)

Salary: NS-35 ($62.40)
