Registered Nurse (RN) - Transitions Team

We are seeking a dynamic and passionate Registered Nurse (RN) to join our Transitions team, providing client-centered care in a collaborative environment.

Job Summary:

As a Registered Nurse (RN) on our Transitions team, you will be part of a diverse care team providing services to clients in the community. You will work collaboratively with an interdisciplinary team to conduct joint care planning, focusing on client-centered goals addressing physical, cognitive, social, emotional, and spiritual domains.

Responsibilities:
  • Provide comprehensive care in an interdisciplinary team setting
  • Act as primary contact for clients when designated as primary care provider/clinician
  • Support clients, families, and caregivers in developing strong self-care strategies and optimizing health through effective health promotion strategies
  • Facilitate regular interdisciplinary team communication and lead discharge and transition care planning discussions
  • Support clients through care transitions by collaborating and connecting them with community support services
Requirements:
  • Registered in good standing with the College of Nurses of Ontario
  • Bachelor's Degree in nursing preferred
  • Canadian Nurses Association (CNA) community nursing certification is an asset
  • Minimum of two years of nursing experience
  • Experience in home/community care, acute care, chronic disease management, health promotion, palliative care, and mental health
  • Demonstrated clinical leadership experience and excellent assessment skills
  • Ability to work independently and in a team environment, with excellent interpersonal and communication skills
