Health Careers in Saskatchewan is seeking a highly experienced Senior Social Worker to join our team as a Clinical Lead in Discharge Planning and Support. This is a full-time temporary position located at the Urgent Care Centre in Regina.
Key Responsibilities- Provide clinical leadership and supervision to a team of social workers in Discharge Planning and Support.
- Assess and support patients in need of discharge planning and support services.
- Develop and implement effective discharge plans that meet the needs of patients and their families.
- Collaborate with interdisciplinary teams to ensure seamless transitions of care.
- Provide education and training to staff on discharge planning and support best practices.
- Contribute to the development and implementation of policies and procedures related to discharge planning and support.
- Three years of recognized clinical experience with demonstrated leadership ability.
- Bachelor of Social Work (BSW) degree.
- Registration/eligibility with the Saskatchewan Association of Social Workers (SASW).
- Strong clinical skills, critical thinking skills, conflict resolution skills, time management skills, decision-making skills, organizational skills, problem-solving skills, and leadership skills.
- Effective verbal and written communication skills.
- Ability to lead and coach others, work independently and as a member of a multi-disciplinary team, and be self-directed, creative, collaborative, and have sound judgment.
- HSAS membership as per the SAHO/HSAS Collective Agreement.
