Summary: Experienced Physiotherapist sought to provide patient-centered care in a community-based setting, working with a multidisciplinary team to assess and treat patients, and engage with patients, caregivers, and other healthcare professionals.

We are seeking a skilled Physiotherapist to join our team at Ontario Health atHome Champlain. As a valued member of our team, you will play a lead role in providing connected, accessible, patient-centered care, and be supported by our collaborative team of over 8,000 regulated healthcare professionals.

Key Responsibilities:
  • Assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers, and other healthcare practitioners.
  • Plan, organize, and direct diverse programs of treatment, counseling, screening, and education for patients in a variety of settings.
  • Establish priorities and treatment goals with respect to identified problems and develop methods of achieving these goals.
  • Maintain accurate clinical records in accordance with Ontario Health atHome Champlain policies and procedures.
  • Function as a member of a multidisciplinary team and maintain liaison with other healthcare professionals.
  • Act as an education and resource person for other healthcare professionals.
  • Participate in committees as approved.
  • Participate in professional development activities and maintain knowledge and ability in the realm of physiotherapy.
  • Maintain supplies and recommend equipment for department use within budget limitations.
  • Interpret the role and objectives of the Ontario Health atHome Champlain Physiotherapy Program in the community.
Requirements:
  • Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario.
  • Related professional experience, normally attained over a period of two years in a Physiotherapy role.
  • Related experience in a similar community-based setting is an asset.
  • Current CPR-C certificate.
  • Fluency in English and French is required.
  • Vulnerable Sector Check completed within the last six months.
