As a key member of our multidisciplinary team, you will provide ongoing mental health and psychosocial client services, promoting quality health care outcomes and ensuring that clients receive a high standard of primary health care.
Job Summary- Provide mental health and psychosocial client services, including screening and assessing client needs and determining relevant treatment plans.
- Initiate or participate in community development initiatives and evaluate PHC initiatives within the site.
- Promote quality health care outcomes, working within established standards of practice to ensure high standards of primary health care.
- Deliver mental health and psychosocial client services, including assessment, treatment planning, and case management.
- Develop and maintain effective treatment relationships with clients.
- Collect, record, and report comprehensive client information accurately.
- Participate in community development initiatives and evaluate PHC initiatives within the site.
- Undergraduate degree or diploma in Nursing or other allied health profession.
- Valid Class 5 driver's license.
- Registered/eligible for registration with the appropriate licensing body.
- Advanced verbal communication skills.
- Advanced written communication skills.
- Ability to work independently and as a member of a multidisciplinary team.
- Sound judgment, tact, initiative, and high standards of integrity/responsibility.
