Job DescriptionThe University Health Network, a leading research and teaching hospital, is seeking a skilled pharmacist to join its team. As a key member of the outpatient pharmacy, you will be responsible for reviewing and verifying medication orders, identifying and resolving drug-related problems, and providing direct supervision to technician staff. You will also be expected to conduct medication reconciliation, follow up on medication incidents, and promote safe medication practices.
Key Responsibilities:- Review and verify medication orders for appropriateness
- Identify and resolve drug-related problems
- Conduct medication reconciliation
- Follow up on medication incidents and promote safe medication practices
- Provide direct supervision to technician staff
- Respond to drug information questions
- Perform other duties consistent with the job classification
- Completion of a university degree in pharmacy or equivalent required
- Certificate of competency from the Pharmacy Examination Board of Canada required
- Active registration with the Ontario College of Pharmacists required
- Certificate of competency from an accredited smoking cessation program and flu vaccination injection program required
- Familiarity with common drug regimens and dispensing processes
- Experience working as part of a multi-disciplinary team
- Strong interpersonal, communication, and customer service skills
- Working knowledge of adherence programs and dispensing processes
- Experience handling pharmacy inventory and manual billings
- Excellent verbal and written communication skills
- Excellent customer service and interpersonal skills
- Excellent organizational and time management skills
