We are seeking a highly skilled and compassionate Pharmacist to join our team at Lawtons. As a key member of our pharmacy team, you will be responsible for providing exceptional patient-centered care and exceeding patients' expectations.
Key Responsibilities:
- Patient Care:
- Provide clinical services, including chronic health condition monitoring and management, medication reviews, health risk assessments, prescription renewals or modifications, minor ailments, or other prescribing services.
- Initiate care plans, follow up, and complete associated documentation.
- Collect relevant patient, drug, and medical condition information.
- Identify and resolve actual and potential drug-related problems.
- Monitor and optimize medication adherence.
- Provide patient counseling tailored to the needs and concerns of the patient to ensure comprehensive understanding of the medication.
- Understand all pharmacy offerings and provide each patient with a personalized interaction and intervention if appropriate.
- Customer Service:
- Provide superior service and exceed patient expectations.
- Acknowledge patients promptly and professionally with warmth, empathy, and compassion.
Requirements:
- Bachelor of Science in Pharmacy or Doctor of Pharmacy from a recognized institution.
- Licensed Pharmacist in the province of responsibility.
- Authorization to Administer Injections.
- Excellent attention to detail, ability to multi-task, and work independently in a fast-paced environment.
- Adhere to and implement all applicable company standards.
