Key Responsibilities- Client Assessment and Care Planning: Conduct thorough assessments of clients with complex geriatric and adult health conditions, and develop personalized care plans to meet their unique needs.
- Health Education and Promotion: Provide education and support to clients and their families on health management, self-care, and wellness strategies to promote optimal health outcomes.
- Interprofessional Collaboration: Work effectively with other healthcare professionals, including physicians, nurse practitioners, and community resources, to coordinate client care and ensure seamless transitions across the healthcare continuum.
- Case Management and Coordination: Manage client caseloads, prioritize care, and coordinate services to ensure timely and effective interventions.
- Documentation and Reporting: Maintain accurate and comprehensive clinical records, and provide regular updates to clients, families, and healthcare teams.
- Education and Experience: Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM), and a minimum of one year of recent related clinical experience in gerontology and adult health care.
- Skills and Abilities: Demonstrated knowledge and skills in gerontology, adult health care, and case management, as well as excellent communication, interpersonal, and problem-solving skills.
