Community Health Nurse

We are seeking a skilled and compassionate Registered Nurse to join our Community Health team at Fraser Health. As a Community Health Nurse, you will work independently in the community setting, collaborating with an interprofessional team to manage client caseloads and provide comprehensive care.
Responsibilities- Establish a therapeutic relationship with clients through interpersonal and interviewing techniques, ensuring their autonomy and dignity in decision-making and care planning.
- Screen referrals, conduct individualized client assessments, and provide interprofessional care planning and interventions, including clinical care and referral services for clients with complex chronic conditions.
- Develop comprehensive shared patient/client care plans in collaboration with the interprofessional team, primary care provider, client, and family, and facilitate the transition of care plans to referring sources and community agencies.
- Provide direct client care, identify other required care services, and maintain clinical records, including intake screens, client assessments, clinical interventions, treatment formulations, care plans, and progress notes.
- Participate in department quality improvement and risk management activities, identify client care issues, and collect data, and participate in research opportunities as required.
Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) is required. One year of recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient care setting is also required.
Valid BC Driver's license and access to a personal vehicle for business-related purposes are necessary.
