Community Health Nurse

Summary: Seeking a Registered Nurse to work in a community setting, assessing and treating complex geriatric and adult patients with chronic health conditions.
The salary range for this position is CAD $41.42 - $59.52 / hour.
As a Registered Nurse with Fraser Health, you will work independently in the community setting, collaborating with an interprofessional team to manage client caseloads, provide assessments, coaching, interventions, and client care services.
- Establish a therapeutic relationship with clients through interpersonal and interviewing techniques.
- Screen referrals, provide individualized client assessments, interprofessional care planning, and interventions.
- Develop comprehensive shared patient/client care plans in collaboration with the interprofessional team, primary care provider, client, and family.
- Provide direct client care, identify other care services required, and provide comprehensive explanations of care to clients and families.
- Collaborate with other healthcare professionals in clinics, community programs, and services to facilitate the flow of information.
Responsibilities include:
- Establishing a therapeutic relationship with clients.
- Screening referrals and providing individualized client assessments.
- Developing comprehensive shared patient/client care plans.
- Providing direct client care and identifying other care services required.
- Collaborating with other healthcare professionals.
Qualifications:
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions.
- Valid BC Driver's license and access to a personal vehicle for business-related purposes.
