About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Markham Stouffville Hospital Corporation. As a key member of our inter-professional team, you will provide clinical pharmacy services to patients across various departments, including Surgery, Medicine, ICU/CCU, Childbirth and Children, Psychiatry, Rehabilitation, Palliative, Complex Care, Oncology, and Emergency.

Key Responsibilities
  • Provide a range of drug distribution and clinical pharmacy services to patients across various departments
  • Participate in patient education and support teaching students
  • Provide drug information to nursing and medical staff
  • Work collaboratively with pharmacists and pharmacy technicians as an integral member of our pharmacy team
Requirements
  • Graduated from an accredited university pharmacy program
  • Licensed under Part A registration status with the Ontario College of Pharmacists
  • Hospital pharmacy residency is preferred
  • Previous experience in a hospital setting with unit-dose and IV admixture programs is required
  • Demonstrated experience identifying, resolving, and preventing drug-related problems to optimize medication therapy and safety
  • Demonstrated computer efficiency is essential
  • Excellent communication and interpersonal skills and the ability to work collaboratively with the members of the interdisciplinary team
  • Evidence of professional development, continuing education, and leadership experience is preferred
  • Demonstrated good attendance and performance records with the ability to maintain these same standards
  • Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital-related policies, health and safety legislation, and best practices and completing relevant mandatory education as required
