Detailed Overview

In collaboration with the Renal Interdisciplinary team and CNE, the Dialysis Transition Nurse facilitates a smooth transition of new clients into dialysis and/or transplantation under Fraser Health's New Starts Program at assigned locations.

This position identifies new chronic dialysis patients and assesses requirements of patients/families in regard to learning, orientation and education needs; referral needs; and psychosocial-emotional assessment. In collaboration with the interdisciplinary team, develops a care pathway to optimize favourable patient outcomes; determines appropriate timing for a variety of dialysis options and/or transplant alternatives; plans and provides support to patients/families awaiting transplantation; discusses, advises and educates patients/families on end of life alternatives and care options. Acts as a resource to patients and the clinic/unit on home dialysis therapies and transplant issues; coordinates/facilitates home dialysis therapy options (Peritoneal Dialysis and Home Hemodialysis); regularly monitors new patients by assessing and evaluating patient progress toward outcomes. Documents and maintains patient information and departmental statistics related to renal statistical requirements. Participates in the development and maintenance of Renal Quality Improvement (QI) program(s); participates on designated clinic-based and professional committees/teams.

Responsibilities

  1. Identifies new chronic dialysis patients and assesses requirements of patients and/or families in regard to learning, orientation and education needs, referral needs and psychosocial-emotional assessment during the first 3 months of dialysis.
  2. Plans, develops and implements an effective care pathway based on patient assessment and evaluation to optimize favourable patient outcomes. Plans and coordinates activities providing support for patients awaiting transplantation; determines appropriate timing for a variety of dialysis options and/or transplant alternatives. Discusses, advises and educates patients/ families on end of life alternatives and care options.
  3. Collaborates with Interdisciplinary Renal team to determine appropriate timing for introducing dialysis and transplantation options to patients.
  4. Identifies potential patient complications and coordinates care for assigned patients utilizing established protocols and procedures in consulting with the integrated renal team. Coordinates referral arrangements for patients requiring specialized treatment options such as pulmonary rehabilitation.
  5. Collaborates with dialysis units to confirm availability of resources to ensure timely initiation of dialysis. Coordinates patient access and involvement in relevant programs/services within their community; promotes and facilitates home dialysis therapies and transplantation to patients/families eligible for these services.
  6. Liaises and collaborates with the interdisciplinary Renal team member to facilitate patient's referral. Coordinates recipient-related activities when a patient is activated from the waiting list for transplantation (i.e. arranges patient entry into health care system, explains process to patient/family.
  7. Provides support to members of the interdisciplinary team regarding dialysis and transplantation options; provides education and acts as a resource to patient and family, coordinating patient access and involvement in relevant programs/services within their community. Reviews and modifies existing educational supports as needed to meet program and patient/family outcomes.
  8. Regularly monitors and follows up with new clients/families during dialysis in order to evaluate the effectiveness of educational and support needs and patient progress towards expected outcomes. Monitors client progress by liaising with referring physician, other health care professionals and the transplant team proving updates on changes in client condition. Triages medical problems as required.
  9. Collaborates with the interdisciplinary Renal team members to facilitate patient's referral to appropriate resources such as Social Work, Dietitian, Pharmacy and Pastoral/Spiritual Care, as required.
  10. Documents patient/family perspectives, nursing observations and activities which support patient/family priorities. Develops and maintains records of patient contacts, conversions to home therapies, referrals, educational activities and outcomes.
  11. Participates with the interdisciplinary team assisting to plan, develop, revise and maintain educational materials for new dialysis patients, including the collegial sharing of information.
  12. Liaises and collaborates with Clinical Nurse Educators to ensure educational, orientation and resource needs of patients/families are followed; reviews and revises care pathways, as required.
  13. Participates with other program team members to establish, implement and maintain a Quality Improvement (QI) Program where indicators are identified, tracked and reported.
  14. Participates on designated clinic-based and professional committees/teams. Attends meetings, provides input into client care documentations; provides input to the improvement of patients and family including the efficient and safe operation of the Clinic.
  15. Performs other related duties as assigned.
Qualifications

Education and Experience

Graduation from an approved School of Nursing, plus certification in a recognized Nephrology program. Two (2) years recent related peritoneal dialysis and/or hemodialysis experience, including experience in progressive renal insufficiency and chronic disease management, or an equivalent combination of education, training and experience.

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

Valid BC Drivers License plus access to a vehicle for business-related purposes.

  • Ability to communicate effectively both verbally and in writing.
  • Ability to teach and provide patient/family education re; renal processes related to dialysis/transplantation.
  • Ability to deal effectively with others, within an interdisciplinary environment.
  • Ability to work independently and collaboratively within a team-based environment.
  • Ability to operate related equipment including related software programs.
  • Physical ability to perform the duties of this position.
