BC Mental Health & Substance Use Services is seeking a highly skilled and compassionate Forensic Community Liaison Nurse to join our team. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to individuals with complex mental health and substance use challenges.
Key Responsibilities:- Conduct comprehensive biopsychosocial assessments to identify client needs and develop individualized treatment plans.
- Provide community case coordination and advocacy services to ensure seamless care transitions and effective communication with mental health teams, private psychiatrists, and other stakeholders.
- Maintain accurate and concise documentation of client information, including progress notes and treatment plans.
- Collaborate with other healthcare professionals to provide holistic care and support to clients and their families.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses & Midwives (BCCNM).
- Two (2) years recent, related experience working with individuals in the community with severe and persistent psychiatric illness.
- Valid BC Drivers License.
