Outreach Support Worker, Indigenous Community Services

We are seeking a highly motivated and compassionate Outreach Support Worker to join our team at Fraser Health Authority. As an Outreach Support Worker, you will play a vital role in providing support services to Indigenous clients struggling with homelessness and living with substance use/mental illness/chronic medical concerns.
Key Responsibilities:- Provide outreach community-based support services to Indigenous clients, including teaching life skills, emotional and social support, advocacy, information, and resources.
- Collaborate with the client and other members of the assigned team to meet individual needs and situational requirements.
- Facilitate connections to Indigenous cultural services and provide culturally safe and competent services in a variety of environments.
- Document client progress and maintain permanent records of client activity.
- Participate in providing substance use treatment, home finding and general housing support, individual and personal care services.
- Provide individual vocational supportive counseling and other work-related supportive services.
- Diploma in Community Social Services, e.g. Addictions Support Worker, Mental Health Worker, Community Health Worker, or Social Services Worker inclusive of counselling skills training.
- Two (2) years recent related experience working with individuals with mental illness/substance use issues, including teaching life skills, experience with Indigenous culture and practices, cultural safety and practices in Indigenous community services.
- Current CPR certificate. Current Food Safe certificate. Valid Class 5 BC Drivers Licence and access to personal vehicle with appropriate business insurance coverage.
