Job Title: Seniors Community Connector

Job Summary

The Seniors Community Connector plays a vital role in bridging the gap between healthcare and social care. This position oversees the operation and delivery of the Social Prescribing Program, connecting seniors with community-based support services and programs in Abbotsford. The Community Connector works with older adults to develop personalized care plans, provide practical support, and establish relationships with community organizations. This role requires experience, training, and interpersonal skills to make meaningful connections and discern the unique needs of seniors being referred.

Key Responsibilities

- Conduct assessments and co-develop personalized care plans with seniors and their families
- Provide practical support to older adults and their families
- Establish and maintain relationships with community organizations
- Connect seniors with community-based support services and programs
- Develop and maintain partnerships with local healthcare professionals
- Monitor client progress and ensure smooth transitions during program discharge

Requirements

- Diploma in social work, social services, or a related field
- Strong knowledge of seniors' issues and challenges related to healthy aging
- Experience working with seniors and diverse populations from different cultures and socio-economic backgrounds
- Knowledge of community resources, programs, and services available to seniors
- Strong verbal, written, and active listening skills
- Strong networking and relationship-building skills
- Strong planning, organization, and time management skills
