About the Role

VON Canada is seeking a skilled Personal Support Worker to join our team as a Care Technician in Palliative and Acute Care. As a Care Technician, you will provide direct, hands-on care to clients in their homes, working collaboratively with our nursing and Home Care programs.

Key Responsibilities
  • Observation and Documentation: Under the direction of a Registered Nurse, you will observe and document clinical data in the patient's electronic medical record as per VON guidelines.
  • Personal Care: You will provide personal care to support activities of daily living and medication administration, under the direction of the Registered Nurse.
  • Communication and Collaboration: You will be in direct contact with the Registered Nurse through a virtual real-time electronic monitoring system, and contribute to the development and modification of a patient's care plan based on patient outcomes observed and reported directly to the Registered Nurse.
  • Resource Coordination: You will identify if additional resources are required in the home, such as home support, and coordinate required needs with the eShift office on scheduling.
  • Client and Family Communication: You will communicate and collaborate with the family/care giver of the patient to ensure a positive patient experience, under the direction of the Registered Nurse.
Requirements
  • Education and Certifications: Personal Support Worker (PSW) certification, Developmental Support Worker (DSW) certification will be considered.
  • Experience: One to two years' experience as a PSW in Palliative/Pediatric or acute care (hospital).
  • Skills: Ability to learn and demonstrate competency for additional hands-on clinical skills required to support a patient's activities of daily living, ability to adjust to working in different social and economic environments, awareness of community resources, strong written and verbal communication skills, and demonstrated basic observation and problem-solving skills.
Work Conditions and Physical Capabilities

You must have access to a personal vehicle for business use and possess both a current driver's license and proof of vehicle insurance. You must also be willing to travel to neighboring communities and have a current criminal background check including a vulnerable sector screen.
