Registered Nurse - Transition Line (Specialty Nursing) Fraser Health is seeking a skilled Registered Nurse to join our team in the Transition Line (Specialty Nursing) department at Burnaby Hospital. As a key member of our interprofessional care team, you will provide high-quality patient care, consolidate your skills, and participate in post-graduate specialty nursing courses. **Job Summary** We are looking for a dedicated and compassionate Registered Nurse to provide direct patient care, assess, plan, provide, and evaluate nursing care to patients. You will work collaboratively with the interprofessional care team to deliver person-centered care, promote patient autonomy, and advocate for patient rights. **Responsibilities** • Assess patients and collect information from various sources to make informed nursing diagnoses and develop effective care plans • Collaborate with patients and their families/caregivers to develop and implement care plans, promoting patient-centered care and autonomy • Participate in the care and discharge planning process, ensuring comprehensive care planning and delivery • Coordinate nursing care, prioritize tasks, and delegate responsibilities to support staff • Collaborate with the interprofessional care team to identify and resolve issues, adjust care plans as needed, and address changes in patient condition • Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance to support self-care and discharge plans • Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures **Qualifications** • Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) • Completion of the applicable post-graduate specialty nursing course/program and/or willingness to complete the applicable post-graduate specialty nursing courses/program • Current certification in Basic Cardiac Life Support (BCLS) **Skills and Abilities** • Ability to complete initial and ongoing patient assessment and provide nursing care through appropriate/prescribed technical, therapeutic, safety-type interventions • Ability to communicate effectively with patients, families/caregivers, and the interprofessional care team using verbal, written, and electronic communication means • Ability to promote person-centered care, demonstrating empathy, sensitivity to diverse cultures and preferences, and patient advocacy • Ability to integrate and evaluate data from multiple sources to problem-solve effectively • Ability to teach and instruct patients and families/caregivers about topics essential to their health care and well-being • Ability to organize and prioritize work • Ability to collaborate with and promote cooperation among members of the interprofessional care team • Ability to use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence-informed practice • Ability to operate related equipment, including relevant computer applications • Physical ability to perform the duties of the position **About Fraser Health** Fraser Health is a leading healthcare provider in Metro Vancouver and the Fraser Valley, serving over two million people.
